Job Overview
We are seeking a dedicated and compassionate Primary Care Nurse Practitioner to join our healthcare team. Duties
- Conduct thorough patient assessments to evaluate health status and identify potential health issues.
- Develop and implement individualized care plans based on patient needs and preferences.
- Manage a range of ailments, providing treatment and education to patients regarding their conditions.
- Address behavioral health concerns by integrating mental health assessments into primary care.
- Utilize your knowledge of anatomy to perform physical examinations and interpret diagnostic tests.
- Perform procedures such as catheterization when necessary.
- Provide hospice palliative medicine services to enhance the quality of life for patients with terminal illnesses.
- Administer esthetic laser treatments as part of patient care when applicable.
- Engage in case management to coordinate care across different healthcare settings.
- Offer health coaching to empower patients in managing their health effectively.
- Collaborate with laboratory services to order and interpret diagnostic tests as needed.
Requirements
- Active Nurse Practitioner license in the state of practice.
- Strong skills in patient assessment and management of various ailments.
- Knowledge in behavioral health practices is preferred.
- Proficiency in anatomy and the ability to perform necessary medical procedures, including catheterization.
- Experience or training in hospice palliative medicine is a plus.
- Familiarity with esthetic laser treatment techniques is desirable but not mandatory.
- Excellent case management skills with a focus on patient-centered care.
- Ability to provide effective health coaching and education to diverse patient populations.
- Strong communication skills, both verbal and written, with an emphasis on empathy and understanding.
