We work with licensed therapists and other Mental Health professionals to provide federally qualified health centers (FQHC) with reliable and efficient access to high quality Behavioral / Mental Health care.
Summary :
The Behavioral Health Therapist will provide outpatient mental health services including psychotherapy using evidence-based practices as well as case management to eliminate mental health barriers to employment and enhance emotional well-being in order to achieve financial security, stability and self-sufficiency.
Responsibilities :
- Conduct assessments including biopsychosocial; collaborate to develop treatment plans.
- Collaborate with probation, other practitioners, families, other collaterals. Develop relationships with local resources appropriate for clients.
- Maintain clients' file in EMR. Documentation must be completed and maintained timely and accurately and in accordance with all applicable standards and agency guidelines / rules. Integrate psychiatric, psychological, and medical needs for clients as needed.
- Interface with all adjunct services related to the treatment of assigned cases.
- Assume primary responsibility for oral and written communications with groups, supervisor, treatment team, adjunct services, family, etc.
- Complete reports as needed.
- Attend team meetings, and supervision meetings.
- Participate in in-service trainings.
- Maintain a minimum of 12 patient seen daily case load
- Interviews patient and family members; prepares psychosocial assessments, addressing all pertinent issues of participants.
- Evaluates data and works with a care team to develop a single plan of care that supports the patients overall health and well-being.
- Educates and assists patient / family in understanding and adjusting to the diagnosis / disability, treatment plan and medical recommendations.
- Provides age appropriate and culturally sensitive counseling and individual / family treatment as well as assistance in family life crisis situations.
- Prepares and maintains all pertinent records, statistics and progress notes.
- Assists in the development of various program curricula.
- Works with outside agencies to ensure coordinated care for patients
- Performs all other related duties as appropriate and / or assigned.
