About the Role:
As a Clinic Care Coordinator, you will play a vital role in supporting our patients by:
- Connecting them with local mental health and social support resources
- Collaborating with our medical team to ensure holistic patient care
- Advocating for patients and removing barriers to care
- Developing creative solutions to support patients’ mental and physical well-being
