JOB TITLE: Community Navigator
POSITION SUMMARY:
The Community Navigator will be responsible for providing triage, assessment, and services to families who are referred by the Delaware State Housing Authority (DSHA-DEHAP), who are near eviction, homeless or near homelessness for assistance that will divert the pending crisis of homelessness. The Community Navigator will access the Delaware Housing Assistance Program (DEHAP) and explore all possible options with each family to solve their current housing crisis.
ESSENTIAL FUNCTIONS:
- Knowledge of Delaware Housing Assistance Program (DEHAP)
- Provide a strength-based assessment for each family and engage with families in providing required documentation and information to complete application for DEHAP support.
- Develop comprehensive housing plans with each family which should include clear action steps for each party to follow up.
- Identify other needed services and make referrals as appropriate.
- Assist families in contacting landlords, completing housing applications and other paperwork as necessary.
- Complete a detailed budget with each family and determine what level of funding is necessary for the family to be successfully housed.
- Complete all documentation necessary to access various funding options.
- Meet regularly with Community Navigator Case Manager Supervisor to review cases and to receive guidance and other program information.
- Build and foster relationships with Delaware State Housing (DSHA) and Housing Alliance of Delaware (HAD) and continue to seek additional community partners.
- Ensure that all services are documented (e.g. face to face, phone and collateral contacts) in a timely and thorough manner.
- Document client outcomes and enter client data into database.
- Prepare compliance reports for all funding sources.
- Be able to present, monthly, quarterly, and yearly reporting.
- Must be able to work nights, weekends, and holidays when necessary.
- Perform other duties needed to help drive our vision, and fulfill our mission, that “The poor should never be treated poorly.”
QUALIFICATIONS:
- High School Diploma or equivalent REQUIRED
- Bachelor’s degree in related field PREFEERED
- 3 (three)-5 (five) years of housing search experience and/or case management experience required.
- Ability to work with and set appropriate boundaries with clients who may have mental health, substance abuse, and/or domestic violence issues.
- A strong ability to keep work organized.
- A demonstrated ability to work independently.
- Strong problem-solving skills, patience, and perseverance.
- Good math, verbal and written communication skills.
- Computer proficiency in Microsoft Office Programs required.
REQUIREMENTS:
- Willingness to consent to and the ability to pass a drug screening.
- Willingness to consent and the ability to pass an FBI fingerprinting background check, where candidate will assume all cost incurred
