Job Summary
The Systems Transformation Team in the Children and Youth with Special Health Needs Section at the Minnesota Department of Health is seeking a passionate, equity-driven, and innovative individual to serve as our Coordinated Systems Specialist. The individual in this position will leverage advanced public health nursing expertise to drive transformative change, ensuring systems of care are comprehensive, family-centered, culturally and linguistically responsive, and community-based. Guided by evidence-informed practices and a deep commitment to advancing equity, this role focuses on optimizing care coordination, medical homes, and transition supports to empower children and youth with special health needs and disabilities (CYSHND) to thrive within inclusive and supportive structures and communities.
Responsibilities Include :
This position is eligible for telework applicants who reside in Minnesota, Wisconsin, Iowa, North Dakota, or South Dakota, and is based out of St. Paul, MN. Reimbursement for authorized business travel expenses is provided according to bargaining unit guidelines.
