This temporary Planner Intermediate position supports the Diabetes and Health Behavior Unit of the Health Promotion and Chronic Disease (HPCD) Division in the Minnesota Department of Health (MDH) and develops and implements strategies for disease prevention and management of Diabetes and Arthritis chronic conditions. The position will work closely with MDH staff, community members, health care systems, contractors, and grantees to support regional and statewide health initiatives and assist with development, coordination, and evaluation of clinical-community linkage strategies within the federal grant initiatives.

Responsibilities of the role include but are not limited to:

  • Planning & Development: Assist in creating and executing strategies to increase participation in evidence-based programs.
  • Partnership Building: Collaborate with existing partners and establish new partnerships to advance disease prevention and management efforts.
  • Stakeholder Engagement: Identify and engage key stakeholders and communities facing health disparities to assess needs, plan strategies, and develop tools.
  • Implementation & Evaluation: Coordinate efforts for implementing and evaluating strategies, and disseminate findings to expand successful, sustainable approaches statewide.

This position will be used to fill one temporary vacancy with an anticipated end date of 2/15/28.

Work may be conducted in-person in one of the Minnesota Department of Health (MDH) district offices or as telework for applicants who reside in MN, IA, WI, ND, or SD, based on the business needs of the unit and supervisor discretion. Position location will be determined by the top candidate and will be based out of the closest MDH office location: Bemidji, Duluth, Fergus Falls, Marshall, Rochester, St. Cloud, or St. Paul. Attendance to monthly in person meetings in St. Paul is required in addition to occasional travel for stakeholder meetings across the state. Reimbursement for authorized business travel expenses is provided according to bargaining unit guidelines.

Minimum Qualifications

One (1) year as a professional planner, project manager or business process coordinator for community-based services, health care, local public health, or for a government agency. Experience to include both of the following:

  • Development of Diabetes and/or Arthritis programming such as clinical care improvement, behavior change or self-management education programming.
  • Experience in public health related outreach and engagement activities.

A degree in public health, community health, social work, nursing, medicine, public policy, or other related field may count towards professional experience as follows:

  • Associate's = 6 months
  • Bachelor’s = 12 months
  • Master’s = 18 months
  • Doctoral = 24 months

To be assessed at interview:

  • Ability to develop and maintain working relationships with a diverse group of internal and external partners.
  • Knowledge of theories, principles, and practice of public health, community, and population-based health promotion.
  • Knowledge of the impact that policy, system, and environmental changes can have on the burden of chronic diseases.

Preferred Qualifications

  • Experience in planning principles, methods and techniques including applicable laws, rules, and regulations sufficient to develop major statewide plans.
  • Experience implementing strategies for diabetes and/or arthritis and chronic disease prevention programs.
  • Experience with clinical health care systems and current and emerging clinical care, adult patient treatment models, systems, and policies.
  • Knowledge of programs and state and federal procedures regarding the administration of grants and use of grant funds.
  • Experience in research and evaluation.

Additional Requirements

  • Must successfully pass a background and reference check.
  • This employment does not require a valid driver's license, but it does demand occasional travel, which the candidate will be responsible for meeting.
  • The successful candidate under U.S. Citizen and Immigration Services regulations must be able to accept work in the U.S. by the day employment begins. The agency is unable to offer H-1B sponsorship.
