The following counties have vacancies: Faribault & Martin Human Services Counties, deadline to apply is March 13, 2025. Merit System Class SpecificationPrimary goals are to assure that all members receive the most appropriate service necessary to meet their needs, experience smooth transitions between settings of care, and that communication between the health plan, County staff and providers occur when needed in support of clients. Desired outcomes are to promote access to primary and preventive care through collaborative problem solving approaches. This position will work to assure a collaborative approach between health plans, providers and County team members. The position will develop positive relationships with and between health plan staff, County staff, local health care providers, nursing facilities and clients. They will help to assures timely and accurate communication between team members. This position will serve as the health plan expert within the community/County working toward positive outcomes for the client and the health plan. The position promotes preventive services and early intervention to members.

Merit System Examples of DutiesSchedules team meetings to review health screenings, new referrals and care coordination activities.Provides appropriate outreach, screening, assessment or advocacyPerforms personal follow up contact with clientProvides home visits or ongoing short term care coordinationPerforms outreach contacts to current clients to obtain Health Screenings, provide client education and promotes the use of preventative services to improve client health status related to condition managementProvides support at the local level through education of county staff and providers regarding programs and services available and basic operational information.Assists with Quality Improvement Projects and Performance Improvement Projects including outreach activities, data collection and monitoringProvides basic technical support and training to other county staff.
