Classification Summary

Freeborn County Department of Human Services is hiring a full time (40 hours per week, Monday - Friday 8:00 am - 5:00 pm) Social Worker-Child Protection Child Welfare Social Worker/Case Manager.

The Social Worker position provides professional level counseling and consultation, investigates abuse and neglect of children and adults, assesses and assists persons with developmental and mental disabilities, and administers a variety of social services to Freeborn County residents.

Direct Service Duties and Responsibilities
  • Carries a complex caseload with frequent contact with clients.
  • Conducts complex case evaluation, assessing, safety planning, and service referral.
  • Documents and maintains data entry, case plans, assessments, and court reports.
  • Collaborates and participates with community service agencies, school, court, residential
and group settings.
  • Performs other duties as assigned.
  • Meets with clients to provide on-going support, counseling and assessment to assist in
determining needs and social services needs of children with mental health difficulties,
which includes: interviewing and assessing the social service needs of a variety of different
clients; screening clients for program eligibility; and referring clients to a variety of support
services.
  • Collaborates with other community agencies and resources and refers clients to various
service providers such as foster homes, mental health center, residential treatment center,
hospitals and drug treatment centers which includes: identifying available and developing
new resources; coordinating successful services for clients and their families; maintaining
contact to monitor client progress and ensure needs are met; and assisting clients to
arrange transportation.
  • Provides on-call child protection services outside of normal work schedule.
  • Visits children in out-of-home placement monthly, which can include clients out of town,
county or state.
Administrative and Additional Duties and Responsibilities
  • Develops case plans and out-of-home placement plans with obtainable goals, assesses client.
progress in various programs, and evaluates service providers by conducting site
inspections. Determines goals with clients; documents client performance by maintaining up to date and accurate case notes and functional assessments; and monitors and evaluates
the quality and appropriateness of client treatment.
  • Prepares and maintains a variety of records and paperwork related to case management
including reports on site visits, interviews, and reports on abuse and neglect which includes
informing appropriate persons and agencies and making recommendations.
  • Organizes and provides education, community outreach services and public relations.
  • Determines needs, develops formats and arranges locations. Provides information to the
media; speaks to groups about department programs; coordinates programs and groups
such as parent/teen group, life skills program, parent surrogate program, nurturing program and volunteer advocate duties and training.
  • Collaborates with other County departments, including Courts and Law Enforcement, to
investigate problems and determine needs of clients. Petitions the court for custody,
protective supervision, or permanency, implements court orders and reports to the court
  • Collaborates with other county departments to investigate problems and determine needs in the overall administration of social services and mental health services to community
members including court petitions for civil commitment or guardianship.
  • Maintain training/educational competencies and own clinical professional development.
While these are the primary focus of the position, we believe strongly in teamwork and employees will be called upon to perform a variety of duties as part of their role with the County.

Minimum Qualifications

  • A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field.

or

  • A bachelor's degree from an accredited four-year college or university with a major in any field and one (1) year of experience as a social worker in a public or private social services agency.

Examination Process

The examination will consist of a rating of training and experience based on the information you provide in your application and your responses to the supplemental questions included in the application process. You must read the questions carefully, answer truthfully, and include any experience claimed in your answers to the supplemental questions in the Work Experience section of your application. Any experience claimed in your answers to the supplemental questions but not included in the Work Experience section of you application will not be credited. You will be asked to verify your degree with official transcripts upon hire.
