LICENSED PRACTICAL NURSE
BASIC PURPOSE:
Provides direct patient care utilizing the nursing process. Identifies and meets the patients needs under the direct supervision of a Registered Nurse in compliance with hospital policy and procedure. Administers, maintains, and terminates IVs and IV medications as specified under the direct supervision of a Registered Nurse in compliance with hospital policy and procedure.

PHYSICAL REQUIREMENTS:
1. Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling are necessary body movements utilized in performing nursing duties throughout the work shifts.
2. Must be able to lift, move, and position patients of all weights, with varying amounts of assistance in a fair manner on a regular and recurring basis.
3. Must be able to read and write legibly in English.
4. Hearing within normal range is required (i.e., to assess breath sounds, bowel sounds, apical pulse, monitors, etc.)
5. Visual acuity must be within normal range.
6. Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work.

Job Duties
ELECTONIC HEALTH RECORDS (EMR)
- Demonstrates capabilities of use of computers and EMR, documenting required data, vital signs, health problems, med list chronic disease measures, call back and e-forms
MEDICATION ADMINISTRATION
- Administers medication as ordered by the physician and in compliance with hospital policy and Medication Administration Standard
- Reports finding of discrepancies to Pharmacy and Nurse Supervisor/Director of Primary Care
- Administers IV medications according to policy
- Recognizes level of pain/comfort and administers medication per order of physician according to policy
ADMISSIONS
- Works with care team in order to facilitate admission standard
- Provides report to receiving unit
DISCHARGE
- Works with care team to facilitate discharge: Assists the patient/family in scheduling tests, making referrals, preparing patients.
- Assess educational needs and provides resources to meet patients needs.
TRANSFER
- Works with care team in order to facilitate transfer in compliance with transfer standard
- Assists the patient/family in preparing patient for transfer.
- Communicates to transport team, type of transfer and prepares transfer.
- Documents transfer and appropriate EMTALA requirements.
- Notifies receiving unit for acceptance and provides report.
CHRONIC DISEASE MANAGEMENT PROCESS: MODEL FOR IMPROVED OUTCOME
- Perform screening of chronic disease in conjunction with Primary Care Provider utilizing guidelines set forth by Health Disparities Collaborative and MHHS.
- Maintain patient electronic file, recording in a timely and comprehensive manner, all relevant care information and updating the Primary Care Provider, as needed, regarding a patients condition and plan of care.
- Documentation will include all care management interventions provided, updating patients electronic health records including but not limited to: medication list, education, chronic disease record, Self Management goals, lifecycles/health maintenance and follow up.
- Carry out standing orders set forth by the Primary Care Providers to maintain compliance with established guidelines to meet the patients needs.
- Participate in education and/or activities that are available to maintain chronic disease management and improved health outcomes for the patient.
- Individual performing the principle duties of Care Manager must have access to and protect the confidentiality of patient health information necessary to conduct patient reviews, prior authorization determinations, care management, data review and analysis.
- Facilitates and/or participates in team huddles as a team effort in meeting the patients needs, taking a proactive approach and dealing with changes in scheduling, staff, etc.
V THERAPY
- Initiates and discontinues peripheral lines according to policy. Maintains integrity of IV site while performing patient care. Reports observed problems to RN.
- IV Certification necessary with annual validation.
ASSESSMENT/OBSERVATION
- Reports observed changes in patient condition to provider and Clinic RN Coordinator.
- Reports abnormalities in vital signs to provider and RN Coordinator.
- Assists with all aspects of patient care, using assessment and clinic skills.
- Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, communication, safety, education level and related criteria for the adult, geriatric, and pediatric patient per the established age-specific standard.
DOCUMENTATION
- Assist patient/family with completing health history as needed.
- Documents fully and accurately in EMR
- Provides appropriate documentation of nursing intervention, patient response, effectiveness, complications and other pertinent information.
- Assists RN with reports and data for quality improvement.
PATIENT TEACHING
- Anticipates teaching needs, provides patient teaching, and reinforces information provided by RN.
- Assists the other staff in health teaching of the patient, family, and/or significant other as it relates to disease process, treatment, and follow-up care.
- Utilizes patient education resources available and refers as needed.
BASIC PATIENT CARE
- Uses nursing process including assessment, planning, implementing, and evaluation to meet the patients physical and psychological needs.
- Provides a clean, safe, calm, quiet, and comfortable environment per the basic care standard.
- Ensures privacy and demonstrates concern for dignity of patient in performing all patient care.
- Completes assignments in a timely manner.
- Works as a team member according to the standard.
COMMUNICATION
Serves as patient advocate, acting as a liaison between patient, family, and the provider/RN to provide continuity of patient care.

Qualifications
QUALIFICATIONS:
1. Graduate of School of Nursing for Licensed Practical Nursing
2. Must be currently licensed to practice in the state of West Virginia.
3. One year experience as an LPN preferred or with satisfactory performance evaluations.
4. Must successfully complete 90 day orientation as an LPN with satisfactory 90 day performance evaluation.
5. Maintain current CPR certification. Recommend annual CEUs in Pediatrics, chronic disease and /or prevention.
6. Possess a general understanding of care management principles and practices for clients in a primary care setting.
