Job Description
This is a professional position responsible for working with youth referred by juvenile court, local school districts or child welfare agencies for mental health coordination. Short term wraparound/case management services are provided by the clinician.
This is a full-time, benefited position, working out of the James Street Campus in Holland. Typical working hours are Monday-Friday, 8:00am-5:00pm. Regular travel within the county as needed.
Essential Job Functions
The essential functions of this position include, but are not limited to, the following:
- Screen youth, regardless of insurance, for behavioral health needs. Refer youth and families to appropriate community resources including mental health, physical health, mentoring, et cetera.
- Serves as a primary contact for referring agencies.
- Provides wraparound/case management services to support high risk youth and families.
- Collaborates effectively and efficiently with other community agencies.
- Provides education and/or training to community members regarding youth mental health services.
- Attends and participates in agency meetings as well as program specific team meetings.
- Attends and provides report updates to System of Care governance board.
- Collects data, documents efforts, and participates in grant reporting, as applicable.
- Coordinates efforts with Family Services Program Director.
- Performs other duties as assigned.
Required Education, Training and Experience
Master's degree in social work, psychology, or a related mental health field is required. Lived experiences with mental illness, developmental disabilities, or substance use disorders are valued.
Licenses and Certifications:
Must possess or obtain State of Michigan licensure as a Licensed Master's Social Worker, Limited Licensed Master's Social Worker, Licensed Psychologist, Limited Licensed Psychologist, Licensed Professional Counselor, Limited Licensed Counselors, or Temporary Limited License Psychologist. Candidates who are close to obtaining the required licensing may be considered.
Must possess a valid driver's license, with ability to travel throughout the county using own vehicle. Mileage will be reimbursed according to County and IRS guidelines.
Additional Requirements and Information
Required Knowledge and Skills:
- Working knowledge of DSM 5 and the ASAM criteria to screen youth and ability to identify needs in family systems.
- Excellent ability to engage in public speaking and train others.
- Knowledge of community resources to meet the needs of youth and families.
- Good understanding of agency processes, procedures, and quality assurance principles.
- Engages consumers easily, excellent interpersonal communication skills.
- Excellent verbal and written communication skills.
Physical Requirements:
Must be able to perform essential job functions with or without reasonable accommodations, including, but not limited to, visual and/or audiological appliances, and devices to increase mobility.
