Your Responsibilities Include :
Providing personal care based on patient assignments and promptly addressing their needs.
Ensuring patient safety and using equipment properly, including foot stools and side rails.
Assisting with personal care tasks such as bathing, oral hygiene, and changing linens as needed.
Helping patients dress and undress as assigned.
Planning, preparing, and assisting with nutritious meals, including shopping and feeding patients when needed.
Taking and recording vital signs as necessary, demonstrating competency in those skills.
Monitoring and caring for patients’ skin to prevent breakdown and discomfort.
Communicating any significant changes in the patient's condition to the Case Manager, as well as noting caregiver dynamics.
Providing respite care for patients, families, and caregivers as appropriate.
Qualifications :
Must be at least 16 years old.
State certification as a nurse aide is required.
Current CPR certification is necessary.
Must be listed on the state aide registry with a clear record suitable for patient care.
Must understand hospice philosophy and be comfortable providing specialized care to terminally ill patients.
