NORTH LAKE CARE CENTER, an 85 bed Skilled Nursing Facility, is seeking PER DIEM PHYSICAL THERAPISTS to join our team!

Pay: $53 - $55 per hour Depending on Experience
Summary of the Physical Therapist (PT):
The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.

Essential Duties & Responsibilities of the Physical Therapist (PT):
- Creates, implements, and executes patient treatment plans.
- Communicates with patients, families, referring physicians, and other members of the team.
- Provides timely written documentation per facility and department requirements and as required by insurance provider to meet licensing, certification, regulatory, and reimbursement requirements.
- Develops and enhances clinical and professional skills through knowledge and professional associations.
- Supports and participates in department operations and development.
- Reports any changes in patient status or needs to Director of Rehab and interdisciplinary team as appropriate.
- Supervises Physical Therapy Assistants with the goal of ensuring optimal patient care and achievement of outcomes.
- Possessing a Physical Therapy degree and licensed in the state of Florida
- CPR Certified
- Physically able to assist individuals
- Moderate level of computer literacy
- Possess strong listening, speaking, critical thinking and reading comprehension skills
- Socially perceptive
- Strong time-management skills
Salary : $53 - $55
