Job Summary and Qualifications
Role Summary : The Community Outreach Specialist II will work with the MHP Care Coordination team to ensure that patients receive the resources and services they need. This position will work with practices to coordinate care and meet network performance goals. The role is also responsible for assisting patients with needed support services to achieve personal goals and optimal health. Additionally, this position will act as a liaison between MHP patients, healthcare providers, and various groups and services in the community.
