Benefits:

- Competitive pay range: $79,000-$88,000/ year (negotiable based on experience)

Qualifications:

- Graduate of an accredited practical nurse or vocational nursing program

- Minimum of one (1) year of experience, preferably in home care or med/surg.

- Current licensure in state and CPR/BLS certificate

- Excellent communication and problem-solving skills

- Ability to meet physical demands of the job, including lifting and standing

Duties:

- Implement care plans initiated by the clinician

- Provide accurate and timely documentation consistent with the plan of care

- Assess and educate patients and their families/caregivers

- Coordinate home health services and communicate needs to other disciplines (HHA, OT, PT, MSW, ST, Dietitian)
