As a Medical Social Worker with Healthy Living, you'll have the opportunity to forge lasting connections with patients in the comfort of their own homes. By delivering compassionate care to our most vulnerable populations, you'll directly contribute to enhancing the well-being of patients, their families, and loved ones.

What We Offer:

- Competitive compensation starting at $35-45/hr, commensurate with experience

Qualifications:

- Graduation from a Master’s program in social work accredited by the Council on Social Work Education

- Licensure as a licensed clinical social worker by the Board of Behavioral Sciences

- Minimum of one (1) year of experience in healthcare, with preference given to candidates with home health care experience

- Excellent verbal and written communication skills, along with strong organizational abilities

- Current CPR certification

- Valid driver's license and access to a reliable, insured vehicle in good working order

Responsibilities:

- Conduct psychosocial assessments of patients to understand their needs within the context of their illness and environment

- Develop and implement tailored intervention plans based on assessment findings

- Maintain accurate clinical records for all patients referred to social work services

- Offer information and referral services to patients, families, and caregivers regarding practical and environmental needs

- Serve as a liaison between patients, families, caregivers, and community agencies to facilitate access to support services
