Key Responsibilities
Provide program orientation to patients, families, and caregivers, including explaining case manager's role, rehabilitation philosophy, continued stay and discharge criteria, Medicare and insurance benefits, grievance procedures, treatment plan process, and rights and responsibilities.
Coordinate patient/family/caregiver education, promote participation in team discussions related to plans, goals, and status through family conferences and other interactions.
Ensure implementation of patient's treatment plan, supporting patient's strengths, abilities, needs, and preferences, and facilitating involvement throughout the rehabilitation process.
Document findings of Discharge Planning Evaluation (DPE) and psychosocial assessments, communicate social, financial, or discharge needs and preferences of patient/family/caregiver.
