Community Health Worker - Mobile Clinic Program Champion

We are seeking a compassionate and skilled vehicle Driver / Operator to join our team and make a meaningful impact on the health and well-being of our patients. The ideal candidate will be responsible for driving the mobile clinic to various locations, assisting with patient check-in, and ensuring the smooth operation of the clinic on wheels. This role combines clinical duties with the unique responsibility of managing a mobile healthcare unit. General Tasks Mobile Clinic Operations : Safely drive the mobile medical clinic to various pre-arranged locations within the community. Ensure the vehicle is stocked, organized, and operational, reporting any mechanical or equipment issues promptly. Work with clinic staff (nurses, doctors, social workers, etc.) to set up the clinic and ensure it is ready to serve patients. Maintain accurate records of visits, including the clinic's schedule, locations, and patient flow. Perform pre-trip and post-trip vehicle inspections. Maintain all the functions of the mobile clinic for daily operations. Ensuring patient safety in and around the vehicle. Operating the vehicle safely adhering to all traffic signals, speed limits, laws and navigating all traffic with caution. Properly store vehicle after deployment, checking and maintaining fuel, water tanks, propane, and on-board generator. Clinical Support : Prepare examination room and ensure proper sanitization. Ensure compliance with all health and safety regulations. Capture patient vitals with enhanced automatic vitals machine. Support patients with telehealth conference calls. Support Community Health Worker to connect patients to resources, as needed. Safety and Compliance : Ensure adherence to all safety protocols, including proper handling of medical supplies. Comply with all regulations related to safety, transportation, and mobile healthcare standards. Assist in maintaining the cleanliness and hygiene of the mobile clinic. Maintains detailed maintenance logs, tracks fuel usage, schedules repairs, and works with mechanics. Physical Requirements : Ability to drive for extended periods. Capable of lifting 50 pounds and assisting in setting up mobile clinic equipment. Comfortable working in a variety of environments, including rural and underserved areas. Working Conditions : Travel to various community locations, often in underserved or rural areas. Comfortability entering patient homes with mobile health team. Flexible working hours, including some evenings or weekends, depending on the clinic's schedule. High school diploma or equivalent; a degree or certification in healthcare, community health, or a related field is a plus. Valid driver's license with a clean driving record; ability to drive a large vehicle (mobile clinic). Ability to pass a criminal background check. Proven experience in community health, patient outreach, or a related field is preferred. Ability to communicate effectively with diverse populations and build rapport with community members. Strong organizational skills and the ability to multitask in a fast-paced environment. Ability to work independently, as well as part of a team, in both indoor and outdoor settings. Work Environment The Mobile Clinic will provide care for patients primarily in the Monroe County, New York area. Surrounding counties will be served, as needed. Work will be a mix of community travel and onsite in a traditional office setting. Salary & Benefits 80 PTO hours available after first 60 days of service. Medical insurance - 100% employer contribution, participating plans. $10K Life insurance - 100% employer contribution. Dental / Vision - employee contribution. 401K - 120 days eligibility is fully vested with up to a 4% match. Weekly payday, every Friday. Quarterly Profit sharing, Open Book Management organization. Salary : $60,000 - $65,000
