Community Health Worker - Mobile Clinic Program

We are seeking a compassionate and skilled Community Health Worker to join our team and make a meaningful impact on the health and well-being of our patients. The ideal candidate will be responsible for driving the mobile clinic to various locations, assisting with patient check-in, and ensuring the smooth operation of the clinic on wheels. This role combines clinical duties with the unique responsibility of managing a mobile healthcare unit. 

General Tasks 

  1. Mobile Clinic Operations: 

  • Safely drive the mobile medical clinic to various pre-arranged locations within the community. 

  • Ensure the vehicle is stocked, organized, and operational, reporting any mechanical or equipment issues promptly. 

  • Work with clinic staff (nurses, doctors, social workers, etc.) to set up the clinic and ensure it is ready to serve patients. 

  • Maintain accurate records of visits, including the clinic’s schedule, locations, and patient flow. 

  • Perform pre-trip and post-trip vehicle inspections. 

  • Maintain all functions  

  1. Patient Engagement and Support: 

  • Greet patients, provide information about available services, and ensure they feel comfortable during their visit. 

  • Assist in patient intake, gathering personal and medical information, and providing basic health assessments. 

  • Offer health education, guidance, and resources on preventative care, chronic disease management, and available local health services. 

  • Help navigate and provide information on accessing additional resources or follow-up care. 

  1. Community Outreach and Education: 

  • Conduct community outreach and educational campaigns to inform residents about the mobile clinic’s services and health initiatives. 

  • Build relationships with local community organizations, schools, and businesses to foster trust and improve engagement with healthcare services. 

  • Assist with collecting data to assess community health needs and improve services. 

  1. Administrative Support: 

  • Record and update patient information and visit details in appropriate digital or paper systems. 

  • Assist with inventory management of medical supplies, ensuring the mobile clinic is adequately stocked. 

  • Help with the preparation of reports on community health trends, clinic visits, and patient outcomes. 

  1. Safety and Compliance: 

  • Ensure adherence to all safety protocols, including proper handling of medical supplies and patient confidentiality. 

  • Comply with all regulations related to health and safety, transportation, and mobile healthcare standards. 

  • Assist in maintaining the cleanliness and hygiene of the mobile clinic. 

Physical Requirements: 

  • Ability to drive for extended periods. 

  • Capable of lifting 50 pounds and assisting in setting up mobile clinic equipment. 

  • Comfortable working in a variety of environments, including rural and underserved areas. 

Working Conditions: 

  • Travel to various community locations, often in underserved or rural areas. 

  • Flexible working hours, including some evenings or weekends, depending on the clinic’s schedule. 

 

 

Requirements & Qualifications

  • High school diploma or equivalent; a degree or certification in healthcare, community health, or a related field is a plus. 

  • Valid driver’s license with a clean driving record; ability to drive a large vehicle (mobile clinic). 

  • Proven experience in community health, patient outreach, or a related field is preferred. 

  • Ability to communicate effectively with diverse populations and build rapport with community members. 

  • Basic knowledge of health education, preventative care, and community resources. 

  • Strong organizational skills and the ability to multitask in a fast-paced environment. 

  • Ability to work independently, as well as part of a team, in both indoor and outdoor settings. 

  • Complete required training to work in an environment that serves a population of people that use illicit drugs. 

  • Training may include and is not limited to Narcan Administration, CPR & First Aid Certified, De-escalation training and all other training modules assigned by the client. 

Work Environment 

100% Onsite   

Salary & Benefits 

• 80 PTO hours available after first 60 days of service. 

• Medical insurance – 100% employer contribution, participating plans. 

• $10K Life insurance – 100% employer contribution. 

• Dental/Vision – employee contribution. 

• 401K – 120 days eligibility is fully vested with up to a 4% match. 

• Weekly payday, every Friday. 

• Quarterly Profit sharing, Open Book Management organization.
