The Nurse Clinic Care Coordinator role is a dynamic position that operates within the interface of physical and behavioral health. This position requires a dedicated professional who is passionate about improving the lives of our patients.
Base Pay: $24/hour. Rate increases with relevant experience and certifications.

About the Role:
As a Nurse Clinic Care Coordinator, you will play a vital role in supporting our patients by:
- Connecting our patients to resources in our community, both mental and physical
- Conducting Medicare Annual Wellness Visits which involves gathering and updating population health measures and coordinating a prevention plan with patients
- Providing education on managing chronic conditions like diabetes and hypertension
- Crisis Intervention for patients experiencing a mental or physical health crisis
- Conducting Transitional Care Management visits, helping patients bridge the gap between their hospital discharge and primary care follow-up
- Help oversee education of Medical Assistants
- Help with coverage on the clinic floor, assisting providers manage their clinic schedules

Qualifications:
Registered Nurse , Licensed Practical Nurse, or Certified Medical Assistant (Current license required)
