Responsibilities:
- Provide ongoing case management in our Lifeset Program which is a model to address the needs of young people age 17-21 who are transitioning out of care and need individualized skill development and support to transition successfully into adulthood.
- Facilitate Family Support Team meetings.
- Develop individualized care (treatment) plans.
- Coordinate individualized services across all settings.
- Coordinate providers that work in the home, school, and community.
- Document support related activities and services.
- Work a flexible 40 hour/week schedule.
Requirements/Qualifications:
- Bachelor's degree in a human behavioral science and 2 years of experience in a related field.
- Or a Master's degree in a related field.
- Valid driver's license, vehicle insurance, reliable transportation, and a good driving record to allow approval to drive a company vehicle.
- Pass background check and fingerprinting.
