Responsibilities:
- Provide ongoing case management to children and families including facilitating monthly meetings and coordinating services across many settings (home, school, community).
- Travel statewide to visit caseload of 8-10 youth weekly where they are placed.
- Communicate with a strength-based perspective using a trauma informed lens accepting people where they are.
- Document weekly contact notes, service or treatment plans, safety plans and court reports.
- Collaborate with team to reach shared goals while also having the ability to work independently
- Work a flexible 40 hour a week schedule.
Requirements/Qualifications:
- Bachelor's degree in a human behavioral science and 2 years of experience in a related field.
- Or a Master's degree in a related field.
- Valid driver's license, vehicle insurance, reliable transportation, and a good driving record to allow approval to drive a company vehicle.
- Pass background check and fingerprinting.
