Job Summary
The Specialist for the Treatment Foster Care Program coordinates TFC cases including conducting home visits, supporting foster parents, holding treatment review meetings and foster parent support meetings, completing assessments and other paperwork, and providing crisis management as needed. Works as part of a treatment team for the client. This position includes traveling to the clients’ foster home and to other appointments.
TFC Specialist will be trained on the NCTSN (The National Child Traumatic Stress Network) toolkit. This is a comprehensive trauma informed method of creating change and helping individuals heal from trauma. TFC Specialist must have a clear understanding of trauma and demonstrate this understanding in their work with youth and resource providers. The ultimate outcome for a trauma informed philosophy is the creation of safe and stable living environments that promote safety, permanency, and wellbeing.
Responsibilities
Responsible for oversight and coordination of the child’s clinical treatment as outlined in the TFC contract.
· Review all information on child and placement and complete all intake paperwork according to policy.
· Provide in-home support and coaching for all levels of Treatment Foster Care Homes, to aid in the development and success of the placement as well as permanency for youth in the program.
· Assure services provided actively involve the child’s family (biological parent(s), adoptive parent(s), foster parent(s), relative caretakers, legal guardians, or adults who play a significant role) in the treatment and care of the child while in treatment foster care.
· Conduct initial and on-going assessments/interview with placement participants.
· Finalize individual treatment plans for program, child, and families upon discussion with program staff, child’s family, and kinship/foster provider.
· Lead Treatment Review meetings on cases.
· Visit with youth and/or family as outlined in program guidelines.
· Monitor progress on each case, and amend treatment plans as needed
· Attend psychiatric appointments, support proper medication management and informed consent.
· Participate in regular supervision and communicate openly about case needs and emergencies.
· Work with FST team to identify transition steps throughout placement.
· Complete all discharge documentation and distribution of final reports.
· Serve as a member of the agency’s on-call team and participate in the on-call rotation
· Compile program outcome data.
· Ensure adherence to all applicable county, state laws and other regulations.
Provide child specific training to TFC homes.
· Provide assistance to the Care Managers in foster home/client-related crisis situations.
· Participate in Family Support Team Meetings, Team Decision Making Meetings, Stabilization Meetings, and other care management meetings when needed.
· Provide information to MACF and FCCM staff regarding the Resource Development Team through attending monthly care management team meetings, all staff meetings and training. Assist in training new employees.
· Participate in the CQI processes.
· Assist in developing monthly newsletter to send to resource families.
· Provide/oversee proper record keeping and documentation for all activities.
· Contribute to team effort by consulting with other professionals; accomplishing related results as needed.
· Help children and families plan for and adjust to transitions.
· Assess risk and protective factors and family strengths and needs.
· Facilitate permanency and family connections.
· Travel to All Staff Meetings, Resource Development Team meetings, Supervision and any other travel deemed necessary to meet the needs of TFC families.
Requirements
1. Master's degree in psychology, social work, counseling, social work administration, or a related human service degree from an accredited school; or
2. Bachelor’s degree in human behavioral science which includes 30 semester or 45 quarter hours either in development of human behavior, child development, family intervention techniques, diagnostic measure, or therapeutic techniques, such as social work, psychology, guidance counseling, drug and alcohol and child development and two years' experience working with children and families of child-placing or residential care agencies.
3. Experience interpreting clinical treatment plans is necessary.
4. Ability to read, analyze and interpret regulations of the Children’s Division and MACF.
5. Ability to write reports, business correspondence, and home assessments.
6. Ability to effectively present information and respond to questions from Care Managers, clients, foster parents, and the general public.
