Missouri Ozarks Community Health – Ava Clinic is currently accepting applications for a full time Community Health Worker, Behavioral Health.
The community health worker (CHW) will screen, support and coach at-risk patients to manage their own health and assist them in navigating the health and social service systems. Community Health Workers (CHWs) function as part of an integrated team of healthcare providers and serve as a vital link to bridge the gap between health care providers, social service groups, and people in need of care. This position will have a focus on assisting patients specific to our behavioral health and substance use programs among other tasks.
Must be able to communicate and assist patients in clinic setting, via phone, and through home/web visits in a professional manner. This position will assist patients with paperwork and forms, securing transportation, and connecting patients with available programs and groups for needed assistance. The CHW will communicate and update providers and care teams on patient progress and needs as well as assist team with patient centered treatment goals. Must be able to build relationships and interact with community partnerships, health, and social service providers.
Must obtain/possess and maintain a current CHW Certification, have a valid drivers license. High School Diploma/GED required. Must pass a drug/alcohol screening before employment can begin.
