MCRHC is a 25-bed critical access hospital, including a Level 4 Trauma Center Emergency Room seeing 3,200 visits annually. About The Job:
Works collaboratively with physicians, staff, and other health care professionals to assist the health coach to provide care coordination across the health care continuum for targeted patients within the physician office setting. Is an integral member of the health care team who works to ensure safety, best practice and high-quality standards of care are maintained across the continuum. Responsible for disease registry activities for the clinic's targeted patient populations. Responsible for assisting the clinic in identifying care opportunities for both populations and unique patients. Success will be measured by the results of the process and outcome performance measures of the population of patients in the clinic.

The salary: CMA starts at $18.34 increases with experience.
LPN starts at $20.38 increases with experience.
Position Qualifications
- Education:
- High School Diploma or equivalent is required
- Graduate of an approved school for medical assistants, certification as a medical assistant is required.
- OR
- Graduate of an approved school for Licensed Practical Nurses preferred and licensed in Iowa is required for LPN role.
- Experience:
- A minimum of six months' work experience in a health care facility performing duties relating to patient care is preferred.
