Job Description : A reputable 700 bed Skilled Nursing Facility located in Upper Manhattan has an immediate need for experienced Licensed Practical Nurses to work Night ShiftsType : HC-ContractShifts : Night Shift : 11 : 30pm-07 : 30amLocation : New York, NYResponsibilitiesOversee responsibility for patient care of Long-term and short-term individualsProvides direct patient / resident care as needed.Responsible for the accuracy and completeness of the unit's medical recordsInitiates and ensures effective interdepartmental communicationResponsible for maintaining an appropriate and safe environmentQualificationsBSN preferred.Valid NYS Professional Licensed Practical Nurse License.At least 6 months of nursing Home experience.Experienced with NYS DOH and Joint Commission standards.Knowledge of MDS and PRI processes preferred.Strong organization, multi-tasking, critical thinking and problem solving skills.Ability to communicate effectively both verbally and in writing in English.Must be service-oriented with excellent interpersonal skills.Excellent documentation skills and computer proficiency including ability to use clinical, email and MS Office applications.Please email : jabbate@hcmmi.com
