Registered Nurse Case Manager
Overview:
We are seeking a compassionate and experienced Registered Nurse Case Manager to join our expanding team. As a Registered Nurse Case Manager, you will be responsible for coordinating and managing the care of patients, ensuring they receive the highest level of quality and efficient healthcare services related to their auto or work accident. This is a vital role in our organization, as you will be working closely with patients, families, and healthcare professionals to develop and implement comprehensive care plans.
Responsibilities:
- Conduct comprehensive assessments of patients' medical conditions, needs, and goals
- Collaborate with interdisciplinary teams to develop individualized care plans
- Monitor and evaluate patients' progress towards achieving their healthcare goals
- Provide education and support to patients and their families regarding their medical conditions and treatment plans
- Coordinate and facilitate communication between healthcare providers, specialists, and other stakeholders involved in patient care
- Utilize managed care principles to ensure appropriate utilization of healthcare resources
- Perform vital signs monitoring and basic medical-surgical procedures as needed
- Assist with discharge planning and coordination of post-hospitalization care
- Collaborate with behavioral health professionals to address mental health needs of patients
Requirements:
- Valid Registered Nurse (RN) license in [state]
- Minimum 1 year of experience in a hospital or clinical setting
- Strong knowledge of physiology, disease processes, and treatment modalities
- Experience in utilization management or case management preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational skills with attention to detail
- CCM preffered
