Seeking a Community Based Nurse Practitioner to join a thriving healthcare organization that is expanding to the Chicago, IL area to provide primary care and support services to underserved populations.
General Job Responsibilities:
- Provide community-based medical and care-coordination services for recent hospital discharges
- Manage a small panel of homebound members who require primary care
- Resolve acute issue in the home and clinic or by directing member to most effective and efficient solutions.
Minimum requirements:
- Primary Care and/or Urgent Care experience needed
- 2 years experience minimum as a NP
- Experience within community-based, home health, or travel roles
- FQHC experience preferred
