- Reporting to the Coordinator of Care Nurse, you will be responsible for seeing patients in their home for revisits based on a skilled need
- You will implement the plan of care and evaluate the treatment
- You will document the visit electronically and completing documentation within 24-48 hours
- NY State LPN (Licensed Practical Nurse) License required
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility required
- Drivers License and Car required
- Bilingual Spanish, Mandarin or Cantonese preferred
Salary : $65,000 - $74,000
