Job Summary:
We are seeking an experienced Registered Nurse to join our team as a Nurse Specialist - After Hours Care. In this role, you will provide critical care and support to patients and their families during after-hours scheduled shifts.
- Respond to urgent visit requests from the triage team, using your expertise to develop and implement individualized care plans.
- Communicate effectively with the on-call team, including the Hospice physician, to address patient needs and establish a safe plan of care.
- Provide education and support to patients and families on medication management, patient safety, and symptom control.
- Document all interactions, including patient assessments, care plans, and progress updates.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
To be successful in this role, you will need:
- Current New York State RN licensure.
- Minimum two years of experience in medical-surgical nursing, community health, or hospice nursing.
- Valid NYS driver's license preferred.
- Ability to obtain national board certification in hospice and palliative nursing (CHPN).
- Strong communication and interpersonal skills.
