What are the responsibilities and job description for the Designated Coordinator position at MN Care Services?
Purpose:
The Designated Coordinator organizes the delivery of in-home services to individuals enrolled in a 245D program according to the federal and state regulations. As well as the policies and procedures of the program. The Designated Coordinator will perform tasks in a way that demonstrates MN Care Services values of effective communication, teamwork, respect, creativity and inclusivity. This position offices in a specific site throughout the metro area.
Essential Functions Duties would include but not limited to:
1.) Audit all new hire packets for site staff
2.) Be sure all staff are equipped with what is needed to perform their essential job duties
3.) Update client logs and trackers monthly and as needed
4.) Ensure schedule for site has been updated and dispersed in a timely manner, ideally 2 weeks in advance for the entire month
5.) Communicate with site nurses about MAR, complete MAR reviews, and ensure medication is ordered in a timely manner
6.) Provide training/support/supervision to all direct support staff/behavior technicians on site
7.) Conduct at minimum, once a month team meeting
8.) Ensure compliance with all 245D paperwork for people served
9.) Ensure compliance with all house documentation
10.) Connect with administrative staff regarding positions posted for vacancies
11.) Connect with administrative staff to schedule interviews for vacancies and locate other peers to attend those interviews
12.) Responsible for communicating with team and being available 24/7 and responding to emails, phone calls, text messages, etc. within a timely manner unless on scheduled PTO/Out of Office.
13.) Approve time punches for staff
14.) Other duties as deemed necessary
Qualifications & Must Provide Proof:
· (1) a baccalaureate degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children;
(2) an associate degree in a field related to human services, education, or health and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children;
(3) a diploma in a field related to human services, education, or health from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older or equivalent work experience providing care or education to vulnerable adults or children; or
(4) a minimum of 50 hours of education and training related to human services and disabilities; and
(5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
· Excellent driving record, reliable car
· DHS Background Clearance
· Prior experience working in a residential program/setting
· 1 years of prior supervisory/Designated Coordinator experience
· Proficient in Microsoft office software
· Good decision making, good time management, communication skills, flexible schedule
· Experience writing 245D support plans
