Shift Type/Shift Times/Hours/Day: Days, 7:00 AM - 7:30 PM, 12
Shift Notes: 7:00 AM - 7:30 PM/36 hours scheduled/24 hours guaranteed/the Client is allowed to cancel 1 shift per week/Must be willing to float to units where experienced
Weekend Requirements: Weekend schedule to be determined by facility
On-Call Requirements: On-call schedule to be determined by facility

- Learning modules must be completed before the start of the assignment.
- Non-billable hours have been adjusted to reflect 16 hours for each assignment.
- Client or Vendor may cancel an Assignment in progress with 7 days' written notice without penalty.
- PLEASE NOTE, THAT THE CLIENT HAS VERY SPECIFIC BACKGROUND REQUIREMENTS THAT MUST BE ADHERED TO AND ATTESTED TO. PLEASE LOOK AT THE COMPLIANCE REQUIREMENTS IMMEDIATELY***
- Banner will not approve RTO within the first week or holidays. Can not exceed 7 days.

- All RTO should be listed on the cover page and must be less than 7 days.
- RTO will not be approved in the first 2 weeks of the assignment.

- Minimum 24 months of current PCU experience - REQUIRED
- Certifications must be American Heart Association OR American Red Cross and active upon submission - MUST BE INCLUDED IN SUBMISSION - REQUIRED
- No Nursys Hits - REQUIRED
- Flu vaccination - REQUIRED EFFECTIVE 9/16 **Please add vaccination status in the submission notes**
- Travel experience - Preferred
- The client Cover Page completely filled out
- Resume with all facilities, facility size, beds, trauma level, and units listed
- Skills Checklist specific to the discipline (must be less than a year old)
- Active License (verification must be less than 30 days old)
- Certifications (must be active at the time of submission)
