Job Title: Care Manager, RN
Working at Home - Delaware
- Position is for New Castle County (open to candidates in this county or within 30 but NOT more than 1 hour commute)
- Must reside in Delaware
- Face-to-face visits will be required. Must be willing to begin visits on week 1.
Job Description:
Responsibilities:
- Travel to members’ homes, nursing facilities, and community settings for face-to-face assessments and telephonic follow-ups, following state and national guidelines.
- Assess, plan, coordinate, and evaluate care for members with chronic and complex health, social service, and custodial needs.
- Coordinate care across various services, addressing physical, behavioral, LTSS, social, and psychosocial needs in a cost-effective manner.
- Facilitate authorization, care transitions (e.g., hospital to home), and continuity of care.
- Educate members and caregivers on health care needs, benefits, resources, and service options.
- Develop and implement care plans with members or caregivers to meet their specific goals and needs.
- Collaborate with health care teams, including LTSS Member Advocates, discharge planners, and providers.
- Assist in overcoming obstacles to optimal care and connect members with community resources.
- Document all services and interventions in the electronic health record, ensuring HIPAA and regulatory compliance.
Required Qualifications:
- Registered Nurse with 2 years of experience in long-term care, home health, hospice, public health, or assisted living.
- 1 year of home clinical or case management experience.
- Experience with Medicare, Medicaid, and managed care.
- Proficiency in Word, Excel, Outlook, and database documentation.
- Reliable transportation to travel within the assigned territory.
- Ability to meet regulatory deadlines and maintain a dedicated home office space.
- Experience with geriatric special needs, behavioral health, home health, and cultural competency.
- Familiarity with electronic documentation systems and cost neutrality.
- Organized with good communication and computer skills.
- Must be willing to travel 2-3 times a week within the state.
Job Types: Contract, Temporary
Pay: $35.00 - $40.00 per hour
Expected hours: 40 per week
Schedule:
