Job Title: Home Health Registered Nurse (RN)

Location: Morristown, NJ (Somerset County)
Duration: 3-Month Contract (Potential for Extensions)
Pay Rate: $63 - $65 per hour (W2)
Schedule: Monday – Friday, 8:00 AM – 4:30 PM (40 hours/week, No Weekends)
Department: Homecare – Union

Job Responsibilities:

• Provide skilled nursing care to patients in their homes, ensuring quality and continuity of care.
• Conduct patient assessments in stable and emergent situations to determine physical, psychosocial, and environmental needs.
• Develop, implement, and evaluate individualized Plans of Care (POC) based on patient assessments.
• Maintain accurate and up-to-date patient records, documenting visits, treatment plans, and progress.
• Educate patients and families on care plans, medications, home safety, and self-care practices.
• Perform venipuncture, IV care, wound care, and pain management as required.
• Supervise and guide Home Health Aides (HHA), Licensed Vocational Nurses (LVN), and State-Tested Nursing Assistants (STNA) to ensure consistent patient care.
• Arrange necessary referrals to additional healthcare providers or services.
• Comply with OSHA infection control measures to protect both staff and patients.
• Monitor patient cases to ensure adherence to third-party payer requirements.
• Serve as a liaison between patients, families, healthcare providers, and administration.
• Participate in interdisciplinary team meetings and provide on-call services when required.
• Perform other duties as assigned.

Required Skills & Qualifications:

• Licensure: Current Registered Nurse (RN) license in NJ – Required
• Education:
• Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program – Required
• Bachelor of Science in Nursing (BSN) – Preferred
• Certifications:
• Current CPR certification – Required
• CHPN (Certified Hospice and Palliative Nurse) certification – Preferred
• Experience:
• 1-2 years as an RN in a clinical care setting – Required
• 2 years of home health experience – Required
• Software Experience:
• Homecare Homebase (HCHB) Software – Required
• OASIS documentation experience – Required
• Additional Requirements:
• Must have a valid driver’s license, reliable transportation, and auto insurance.
• Ability to work independently and as part of a team.
• Strong communication and organizational skills.
• Knowledge of medications, wound care, IV therapy, and pain management.
