Position : Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Shift Type / Shift Times / Hours / Day : Days 7 : 00 AM 7 : 30 PM 12
Shift Notes : 7 : 00 Am7 : 30 Pm 12 hour DAYS
Weekend Requirements : every other weekend
OnCall Requirements : None

Traveler Policy : Candidates Permanent Address (Facility will not consider local candidates 35mile radius restriction)
Dress Code : Matching Scrubs
Further Specifics :
- Nonbillable Hours : 12 hours of initial hours
- OT / Holiday / Call back Markups : 1.1x Regular rates
Supervises and performs direct and indirect resident care as specified by individual resident care plans and physician orders according to federal state and facility rules regulations and policies.
KNOWLEDGE SKILLS AND ABILITIES :
- Experience : Previous geriatric experience preferred
- Licensure : Current RN / LPN licensure from the State of Michigan
- Skills : Must be able to communicate in English both verbally and in writing. Must demonstrate computer literacy including use of computer programs and applications.
Must be CPR trained and possess a current BLS certification.

- References NOT required.
- A skills checklist completed within one year of application for RN specialties is required. All other professions outside of Nursing are not required but preferred.
- Michigan license verification is required for nurses. Please provide LARA and Nursys; if there is a license hit it would be in the travelers best interest to provide a writeup of what happened for the managers review.
- Physical copies of certifications are only required in the profile if listed above.
- Fulltime employees are not eligible for contract hire within six months of employment.
