- Observe each resident continuously alert to changes in mental and physical well-being; notify the Supervisor of pertinent observations and discuss the need to notify the physician; receive Resident Status reports from off-going shift and give on-coming shift a detailed report of each resident’s status and events of the shift.
- Record narcotics on a 30 days basis
- Accurately receive, transcribe, and carry out physician’s orders.
- Administer medications and treatments as ordered by the physician and make proper documentation via a computerized system.
- Perform wound care and know wound vacs.
- Perform regular duties on the unit such as checking appointments for the day; checking Physical, Occupational, and Speech Therapy appointments; checking activity boards; reminding staff of ambulation lists, weights, and blood pressures, and documenting nutritional charts. Monitor LNA documentation and update LNA clipboard; oversight of infection control practices on the unit; ensure that the unit is clean and hazard-free. Following the chain of command, report any problems to the Nurse Manager/Nursing Supervisor. Encourage LNA observations and include them when documenting and reporting.
- Admit, discharge, and transfer residents, completing documentation and notifications as necessary.
- Complete 24-hour report for his/her assigned unit(s); check new orders; complete list of residents on antibiotics, precautions, etc.; maintain consistent communication with the Nurse Manager/ Nursing Supervisor regarding resident issues or concerns with staff; differentiating between acute and non-emergency problems; and attend all mandatory staff/educational meetings and non-mandatory meetings as assigned.
- Communicate with residents’ families to resolve resident concerns and problems notify family members of new orders, and obtain consent when necessary.
- Receive incoming calls and determine the appropriate response required.
- Follow State and Federal Regulations regarding the storage, usage, and documentation of controlled medications including counting at the change of shift; notify the appropriate Supervisor if discrepancies are present.
- 6 months experience in LTC- REQUIRED
- BLS- REQUIRED
- Work History
- Skills Checklist (pertinent to position), current to 1 year
- 1 Supervisory Signed Reference (from within the past two years of employment)
- License/Certifications - Must be uploaded with profile (Current NH license and BLS, 3 years current experience - Facility will not pend license/certs)
