Position: Registered Nurse (RN) - Case Manager

Start Date: 9/23/2024

Duration: 13 WEEKS

Location: Peterborough, NH, 03458

Shift Type/Shift Times/Hours/Day: Days, 8

Shift Notes: 8 Hr Days 40 hrs Guarantee

Weekend Requirements: Occasion weekends as needed / Holiday rotation as needed

On-Call Requirements: No Call

Travel Policy: Candidates' Permanent Address (Facility will not consider local candidates - 50-mile radius restriction)

• Requested time off during this assignment MUST be noted and verified at upload. NO RTO REQUESTS AFTER UPLOAD. NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better). Holiday rotation is required so must be available.
  
  

The Utilization Review RN is accountable for a designated caseload and plans effectively to manage the length of stay, promote efficient utilization of resources, and facilitate payer authorization.

The UR RN/Case Manager

• Facilitates collaborative management of patient care, intervenes as necessary to remove barriers to efficient care delivery,
• Applies process improvement methodologies in evaluating the outcomes of care coordination, and
• Supports and coaches clinical documentation by staff and providers ensuring documentation reflects the severity of illness and intensity of service.
  
  

Submission Requirements

• Computer skills with demonstrated knowledge of Microsoft Office including Outlook and Excel. - Required
• Minimum of five years of acute care nursing experience, one year of case management, discharge planning, and/or utilization review experience - Required
• Knowledge of performance improvement principles, tools, and techniques.
• Case Management certification- Required
• Prior travel experience - Preferred
• BSN - Preferred
