Job Title: Care Manager, RN
Job Summary:
Seeking an experienced RN to assess, plan, and coordinate care for members with chronic and complex needs in home, community, and facility-based settings.
Key Responsibilities:
- Conduct face-to-face and telephonic needs assessments.
- Develop and implement care plans addressing medical, behavioral, and social needs.
- Coordinate care transitions (e.g., hospital to home, nursing facility to community).
- Facilitate authorizations, referrals, and access to resources.
- Educate members and caregivers on healthcare options.
- Ensure services align with care plans and revise as needed.
- Maintain accurate documentation in electronic health records.
Requirements:
- Active RN license with 2 years in long-term care, home health, hospice, or public health.
- 1 year of case management or home clinical experience.
- Medicare, Medicaid, and managed care experience.
- Strong computer skills (Word, Excel, Outlook).
- Reliable transportation for statewide travel (2-3 times/week).
- Excellent communication, organization, and assessment skills.
Preferred:
- CCM, LBSW, LMSW, or LCSW certification.
- Experience with HIV/AIDS, behavioral health, or developmental disabilities.
Job Type: Contract
Pay: $45.00 - $48.00 per hour
Expected hours: 40 per week
