Experience
EXPERIENCE/EDUCATION/QUALIFICATION
- Must have OASIS (Outcome and Assessment Information Set) Experience
- Home Health and Hospice Experience required
- Experience traveling from patient to patient for home visits is required
- Must be able to manage and adjust to changes in schedule
- A current VT LPN licensure is required
- BLS/BCLS certification is required
- Must have driver's license in good standing
