Responsibilities
- Assess a person’s condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence.
- Collaborate with interdisciplinary teams and colleagues to ensure a person’s needs are met.
- Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.
- Utilize a variety of therapeutic modalities to maximize the care provided to clients
EXPERIENCE/EDUCATION/QUALIFICATION
- Home Health experience and OASIS experience are a must as this candidate will case-manage
- No RTO required
- Vermont PT licensure is required
- Basic Life Support certification is required
