About the Role:
We are seeking a highly skilled Licensed Practical Nurse to join our team at Mobile Primary Care. As a Licensed Practical Nurse, you will be responsible for providing exceptional patient care in a variety of settings, including assisted living facilities and patients' homes. Your primary goal will be to ensure that patients receive the highest quality care possible, while also providing education and support to patients and their families.
Minimum Qualifications:
- Valid nursing license in the state of New York
- Experience in home health nursing.
- Ability to read, write, and speak English fluently
- Excellent patient care and communication skills
- Ability to work independently and as part of a team
Preferred Qualifications:
- Experience working in an assisted living facility
- Experience with electronic medical records
- 2-3 years Clinical or Medical Office experience

Responsibilities:
- Administer medications and treatments as prescribed by physicians
- Monitor and record patients' vital signs and medical history
- Assist with patient mobility and daily living activities
- Provide patient education and support to promote healthy lifestyles
- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans
Skills:
As a Licensed Practical Nurse, you will utilize your skills in home health nursing, patient education, and patient care on a daily basis. You will work closely with physicians and other healthcare professionals to develop and implement patient care plans, and provide education and support to patients and their families. Your ability to work independently and as part of a team, as well as your excellent communication skills, will be essential to your success in this role.
