Responsibilities & Qualifications
Conduct thorough initial assessments, develop care plans, and perform routine follow-up evaluations for patients in the rehab and long-term care units.
Work closely with attending physicians, nursing staff, physical therapists, and other healthcare professionals to coordinate comprehensive patient care plans.
Maintain accurate and up-to-date medical records, ensuring compliance with facility policies, state regulations, and billing requirements.
Educate patients and their families on disease prevention, treatment options, medication adherence, and lifestyle modifications to promote overall well-being.
Participate in quality improvement initiatives, clinical audits, and performance improvement projects to enhance patient care outcomes and facility practices.
