Location:
This position is based in Jacksonville, FL.
Your Role as a Home Health Coordinator:
- Ensure all preventive care is documented, coordinated, and completed.
- Conduct on-site Annual Wellness Visits, Advanced Care Planning, and provide chronic care management for geriatric patients.
- Create and maintain detailed care plans to guide patient interactions and care.
- Make monthly phone calls to patients enrolled in Chronic Care Management (CCM) programs.
- Support continuity of care by coordinating with tertiary care providers, managing care transitions, and handling referrals.
- Use HIPAA-compliant computer systems to facilitate patient care.
Qualifications:
- Minimum 1-year experience in Homecare, Skilled Nursing Facility or Primary Care setting
- Minimum 1-year experience in Chronic Care Management preferred
- Knowledge of medical terminology and disease specific interventions
- Computer/EMR system savvy
