The job duties the LCSW will be expected to perform will be the following :
- Provide holistic psychosocial support and behavioral health counseling / therapy to patients.
- Monitor and coordinate community resources for patients with social economic needs.
- Coordinate treatment plans for patients with mental illness or other barriers to improve health outcomes.
- Communicate with team members, providers, patients, and the community in a timely manner to ensure continuity of care.
- Provide excellent customer service.
- Collaborate with patient / family and other team members in coordination of patient care.
- Comply with all medical record standards and policies.
- Provide timely and appropriate documentation in accordance with Medicare guidelines.
- Any other duties as assigned in response to the growth of the company and in recognition of employee strengths
