We're seeking a dedicated Home-Based Primary Care Nurse Practitioner to join our dynamic team. As a crucial member of our healthcare family, you'll have the opportunity to provide personalized medical services to patients who may have difficulty accessing traditional healthcare settings.
Position: Home-Based Primary Care Nurse Practitioner
Location: Hybrid in-person visits and Telehealth
Responsibilities:
- Conduct comprehensive assessments of patients in their homes.
- Develop and implement individualized care plans in collaboration with patients, families, and other healthcare professionals.
- Perform regular patient follow-ups and monitor their progress.
- Provide education and support to patients and their families regarding healthcare management and prevention.
- Coordinate care with other members of the healthcare team, including physicians, social workers, and therapists.
- Utilize telehealth technologies as needed for virtual consultations and follow-ups.
- Document patient encounters and maintain accurate medical records.
Qualifications:
- Current licensure as a Nurse Practitioner in Nevada.
- Certification as a Family or Adult-Gerontology Nurse Practitioner.
- Minimum of 1 year of NP experience
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Comfortable with technology and proficient in electronic health record systems.
- Compassionate and empathetic attitude towards patients and their families.
