Mobridge Regional Hospital & Clinics is seeking a self-motivated team player to join our rural healthcare organization. Successful applicants must have a passion for helping others. We are looking for someone with a strong desire to use their own education and life experiences to serve as an intermediary between health care, social service organizations and the community. Their role will be to act as a non-medical member of the patient care team and advocate on behalf of their patients’ best interests while helping them reach their health goals.
This individual will work closely with patients, families (support network), MRH&C medical providers, primary care team, and social services agencies to facilitate care coordination. As well as a connection to resources that support Emergency Room/EMS patients to improve their health, self-sufficiency, and general well-being. The Community Health Worker will be working with a wide variety of patients from all backgrounds in both clinical and community-based settings.
Primary Job Responsibilities:
A. Health system navigation and resource coordination, such as:
- Working as a patient advocate primarily within the emergency setting.
- Helping patients find providers and assisting patients to make appointments for various services. Including connecting patients with necessary specialty provider appointments as recommended.
- Transporting and/or arranging transportation for patients to get to and/or from medical appointments.
- Possibly attending appointments with patients for medical services.
- Linking patients/clients to other relevant community resources, such as social services, counseling and support groups.
- Assisting with insurance navigation for treatments and medications.
- Coordinating pharmacy pick-up or delivery as well as setting a plan with the patient regarding medication schedule and reminders.
B. Provide appropriate health information and education to patients, such as:
- Providing health education regarding social determinants of health, tailored to each patient which may be impacted. (including foods, family/community traditions, etc.).
- Ensuring patients’ own support network, including friends and family understand health information and education provided.
C. Other skills and abilities:
- Ability to connect with and provide ongoing support in a behavior change process if needed. Will document patient information and progress in the Electronic Medical Records system.
- The Community Health Worker will conduct presentations on health and wellness activities and management of health concerns.
- Ability to work independently without supervision and as part of a team.
- Staff may be asked to assist in the event of a Public Health emergency.
D. Minimum Requirements:
- Minimum of a high school diploma or equivalent.
- Prefer a minimum of two years of applicable experience.
- Will complete a paid 8-week online approved CHW training program in South Dakota and complete other mandatory training, including CPR certification.
- Must have a valid driver’s license and a clean driving record.
- Experience with navigating local medical and social support systems is desired.
- Demonstrated organizational and time management skills
- Applicant must be able to communicate professionally and effectively both verbally and in writing with medical staff, including providers, and other professionals, Social Service agency staff as well as people from all cultural and socio-economic backgrounds.
- Proficient computer skills, including work in MS Office Suite (Outlook, Word, Excel, etc.).
- Ability to pass a thorough background investigation.
This is a full time 40-hour per week position, which could include some evening and week-end work.
Starting wage range will fall between $18.00 - $22.00 Per hour.
