LPN Case Manager HOME HEALTH

Job Overview
We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the necessary services to improve their health outcomes. Duties

• Conduct thorough patient assessments to determine individual needs and develop personalized care plans.
• Coordinate and manage patient services, ensuring that all aspects of care are delivered effectively.
• Perform diagnostic evaluations to identify health issues and recommend appropriate interventions.
• Maintain accurate medical records and documentation for each patient, ensuring compliance with regulatory standards.
• Facilitate discharge planning by preparing patients for a smooth transition from healthcare facilities to home or other care settings.
• Monitor vital signs and assess physiological conditions to track patient progress.
• Collaborate with multidisciplinary teams to optimize patient outcomes and enhance service delivery.
• Provide education and support to patients and their families regarding treatment options and resources available.

Experience

• Proven experience in case management or a related field is preferred.
• Knowledge of managed care systems and patient assessment techniques is essential.
• Familiarity with medical records management and discharge planning processes is highly desirable.
• Strong understanding of physiology and the ability to interpret vital signs effectively.
• Excellent communication skills, both verbal and written, are necessary for successful interactions with patients, families, and healthcare professionals.
• A background in healthcare or social services will be beneficial for this role.
