Job Summary
The Case Manager will be responsible for overseeing and coordinating various aspects of program operations, ensuring the effective delivery of services to individuals recovering from injury, illness, and or disabilities. This role involves managing relationships, case management, care coordination and contributing to program efficiency.
Responsibilities
- Adhere to organizational policies and regulations
- Coordinate program activities to support individuals recovering from injury, illness, and or disabilities.
- Coordinate with program staff, providing guidance and support as needed
- Collaborate with internal teams to enhance program effectiveness
- Conduct administrative tasks to maintain accurate program records
- Transporting clients to their appointments and social service needs regularly.
Experience
- Proven experience in social work, human services, behavioral health, or related field (Preferred)
- Strong background in case management (Preferred)
- Ability to work effectively with individuals recovering from injury, illness, and or disabilities
- Excellent organizational and communication skills
- Demonstrated proficiency in relationship management and team collaboration
- Knowledge of best practices in supporting individuals recovering from injury, illness, and or disabilities
- Case management experience (Preferred)
Job Type: Full-time
Pay: $17.00 - $18.00 per hour
Expected hours: 40 per week
Schedule:
- 8 hour shift
- Day shift
- Monday to Friday
- No weekends
- Weekends as needed
