What are the responsibilities and job description for the Elder Care Coordinator position at Modoc Nation Behavioral Health?
Duties & Responsibilities:
1. Proactively engage elders and possess communication skills/ability to build relationships with elders.
2. Complete needs and safety assessments.
3. Assess elder homes for safety risks and develop a home modification plan to address areas of risk.
4. Complete the assistance application with Elder and keep a record of all applications.
5. Coordinate with contractors for safety home modifications, obtain W9, cost estimates, and invoices.
6. Provide elders with appointment reminders and maintain a calendar for elders to track upcoming medical appointments, therapy appointments, and wellness activities.
7. Identify appropriate community resources and activities to link elders with to enhance wellness (activities, meals, gatherings, healthcare, etc.)
8. Assist in planning and implementing elder-focused events, activities, and meals.
9. Coordinate with MNHS Business Office to purchase/secure assistance items and services.
10. Keep a log of all services provided to elders.
11. Provide daily transportation to elders to appointments, recreational activities, grocery store, etc. and log mileage.
12. Coordinate transport route stops to best facilitate transportation for the greatest number of clients while keeping facility schedules accommodated.
13. Deliver meals to elders as needed.
14. Ensure the transit van is mechanically maintained.
15. Attend outreach events to promote the project.
16. Coordinate with cultural consultants to organize and facilitate cultural activities.
17. Participate in grantee calls and meetings as requested.
18. Provide updates to PCL Project Director.
19. Ensure compliance with HIPAA & other confidentiality regulations in all aspects of job duties.
20. Staff & support outreach activities as required.
21. Attend All – Staff meetings & all other meetings as required.
22. All other duties as assigned.
Qualifications:
1. Must have a high-school diploma or GED.
2. Minimum 2 years experience providing case management or support services.
3. Must have a valid driver’s license with a driving history free of accidents, violations, suspensions, or infractions.
4. Must be able to pass a pre-employment drug screening & background check.
5. Must provide documentation of covid vaccination status or request exemption based on qualifying factors.
Compensation: $30,000 to 41,500 annual salary plus fringe benefits with the opportunity for biannual bonus. Salary is dependent upon education and experience.
Location: This position will primarily operate from Lost River Treatment Center, though travel will be required to off – site locations.
Schedule: This employee shall maintain regular business hours (40 hours per week).
Supervision: This position will operate under the direct supervision of the PCL Project Director and Grant Manager.
