- The Clinical Case Manager provides expert support in patient care coordination, ensuring quality management, continuity,. This role focuses on patient assessment, planning, implementation, and discharge coordination to improve patient satisfaction and clinical outcomes.
- Care Coordination: Oversee patient care from admission to discharge, ensuring proper service utilization and resource management.
- Case Management & Discharge Planning: Integrate case management and social services into care and discharge processes, collaborating with hospital teams and external agencies.
- Quality & Utilization Review: Conduct concurrent medical record reviews and ensure compliance with clinical standards (TJC, CMS, etc.). Educate staff on discharge planning and resource utilization.
- Patient Advocacy: Act as a patient advocate, investigating adverse occurrences and facilitating communication with patients and families about care plans.
- Collaboration with Physicians: Communicate regularly with physicians to optimize patient outcomes and manage costs.
- Policy & Protocol Development: Assist in creating patient care policies and protocols, offering guidance on complex cases.
- Interdisciplinary Collaboration: Lead patient care rounds to review treatment goals, provide family education, and address post-hospital needs.
- Graduate of a Program in Nursing at Bachelors of Science level or higher required
- Certifications: BLS required upon hire
- Arizona Registered Nurse and/or Compact State Registered Nurse required
- Minimum of 2 years as director of Case Management in a critical care hospital required
- The Clinical Case Manager ensures seamless patient care by integrating case management functions with hospital departments, external services, and healthcare facilities, fostering quality outcomes and patient satisfaction.
Salary : $118,000 - $150,000
