You will be responsible for making nursing visits, gathering patient data, instructing patients and caregivers, and documenting care provided. Collaboration with the healthcare team is crucial to promote good health practices and maintain professional knowledge through education and training.
The successful candidate will assist in the provision of nursing care to home health patients, reporting significant changes in patient condition to the Registered Nurse or Supervisor as appropriate. Instructing patients and caregivers according to the plan of care and implementing orders for medications and treatments as prescribed by the physician are also key responsibilities.
